Take-home

A 0.42 hazard ratio for death in extensive-stage small-cell lung cancer. In a disease where we’ve celebrated gains of two to three months, this is a different level.

In the open-label phase 3 DeLLphi-305 trial, 563 patients with extensive-stage SCLC without progression after first-line chemoimmunotherapy were randomized to tarlatamab plus durvalumab or durvalumab alone as maintenance. The median overall survival was 21.1 versus 12.7 months (HR 0.42), and the median PFS 7.4 versus 1.9 months (HR 0.34). Grade ≥3 adverse events occurred in 46.8% versus 40.6%; cytokine release syndrome, mostly grade 1–2, affected nearly half of tarlatamab-treated patients.
