Could biomarkers help us better identify which patients with advanced RCC are most likely to benefit from immunotherapy?
Take-home: Final KEYNOTE-427 analysis found that pembrolizumab monotherapy produced durable antitumor activity in advanced clear cell and non–clear cell renal cell carcinoma, with objective response rates of 36.4% and 26.7%, respectively. Higher PD-L1 CPS and T-cell–inflamed gene expression were associated with improved outcomes. Baseline ctDNA positivity predicted shorter overall survival, while conversion to ctDNA-negative status was associated with better outcomes, supporting biomarker-driven treatment strategies in advanced RCC
